The Southern Metropolitan Regional Palliative Care Consortium (SMRPCC) is an alliance of palliative care services in the Southern Metro Region of Melbourne with the aim to collaboratively implement the Victorian End of Life and Palliative Care framework in the region. The consortium is active in several areas including aged care, disability, progressive neurological conditions and workforce development. Peninsula Home Hospice (PHH) is a not-for-profit community palliative care service providing end of life support to clients and their carers in their homes. PHH acts as fund holder and employer for the SMRPCC. This position is with the SMRPCC
Working for the SMRPCC the Residential Aged Care Facilities (RACF) Project Officer will collaborate with palliative care teams, facility staff and managers, and key stakeholders to implement initiatives towards the improvement of palliative care in aged care as identified in the recent Aged Care report (available upon request).
As part of this collaboration the Project Officer will regularly communicate with current aged care service providers, provide training and assist in the development of sustainable initiatives within facilities.
Selection Criteria
To be successful in this role you will have:
- Current Nurse Registration
- Proven experience in project management within a healthcare setting, preferably in palliative care.
- Strong understanding of the aged care sector and the unique challenges related to palliative care in residential facilities.
- Experience in delivering training
- Excellent communication and interpersonal skills with the ability to engage and collaborate with diverse stakeholders.
